nearest Major Police Station Cost: $45 |

Go to your Service Tasmania Office and ask for an
application form for an Ancillary Certificate. This form also has a
Medical Form involved, so you should take this to your doctor when
having your medical. |

Go back to your Service Tasmania Office with:

1/ Certificate from Road Transport Training. |
| |
2/ A copy of your record of convictions. |
| |
3/ Completed Medical. |
| And say, may I have my Ancillary
Certificate please: Cost $25 |
